Accuracy of venereal syphilis serological diagnosis in Cuban laboratories

Abstract

Introduction: Syphilis laboratory diagnosis is most commonly based on the conduct of serological tests. The World Health Organization requires diagnostic accuracy values above 95% in these tests. It is therefore necessary to evaluate their performance and verify the precision and reliability of the results obtained.

Objective: Indirectly evaluate the diagnostic accuracy of serological tests for syphilis in adults.

Methods: A retrospective descriptive study was conducted of all the documentation concerning external assessment of the quality of syphilis laboratories at Provincial Hygiene, Epidemiology and Microbiology Centers and the Municipal Hygiene, Epidemiology and Microbiology Center of the Isle of Youth, recorded at the National Reference Laboratory in the period 2014-2019.

Results: It was found that laboratories did not participate systematically in external quality assessment. Diagnostic accuracy by Venereal Disease Research Laboratory (VDRL) / rapid plasma reagin (RPR) testing was 91.4% for the laboratories evaluated, whereas accuracy for the Treponema pallidum hemagglutination assay (TPHA) was 96.2%. Of the laboratories studied, 19% (3/16) exhibited accuracy values of 95% for VDRL/PRP and 69% (11/16) for TPHA.

Conclusions: In a proportion of the laboratories evaluated, diagnostic accuracy of serological tests for syphilis diagnosis does not meet WHO requirements, particularly as concerns VDRL/RPR, though it was evident that systematic participation in the external quality assessment program contributes to continuous improvement.
